Clomiphene contains two related compounds: enclomiphene and zuclomiphene.

It has been used off-label for many years to support testosterone production in men with secondary hypogonadism. It may increase testosterone, LH, and FSH without the suppressive effects associated with external testosterone.

Enclomiphene Citrate

Enclomiphene is the component of clomiphene primarily responsible for stimulating the hormonal signals that increase testosterone production.

Some men may prefer enclomiphene because it does not contain the longer-lasting zuclomiphene component found in conventional clomiphene.

The best option depends on availability, treatment goals, laboratory findings, clinical response, and tolerance.

Who May Be a Candidate?

Natural testosterone therapy may be considered for men who:

  • Have low or suboptimal testosterone with low or normal LH
  • Have symptoms consistent with testosterone deficiency
  • Want to preserve fertility
  • Want to maintain natural testosterone production
  • Prefer an oral option over injections or topical therapy
  • Previously used testosterone and want to restore endogenous production
  • Have functional or secondary hypogonadism
  • Are younger and may not be ready for long-term testosterone replacement

This treatment may be less effective when the testes are unable to respond adequately to hormonal stimulation, as can occur with primary testicular failure.

Low testosterone should not be diagnosed from symptoms or a single laboratory value alone.

  • Gastrointestinal discomfort

Visual symptoms or other significant adverse effects should be reported promptly. Treatment may need to be adjusted or discontinued depending on the response.

Natural Stimulation or Testosterone Replacement?

Neither approach is universally better.

Natural testosterone stimulation may be preferable when preserving fertility and endogenous function are priorities. Testosterone replacement may be more appropriate when the testes cannot produce adequate testosterone despite appropriate stimulation or when a patient does not respond sufficiently to oral therapy.
