What Is Hashimoto’s Thyroiditis?

Hashimoto’s thyroiditis is an autoimmune thyroid disorder.

In autoimmune disease, the immune system mistakenly reacts against the body’s own tissues. In Hashimoto’s, the thyroid gland becomes the primary target.

The thyroid produces hormones that help regulate:

  • Energy production
  • Metabolism
  • Body temperature
  • Heart function
  • Digestion
  • Brain function
  • Mood
  • Muscle function
  • Menstrual and reproductive health

As thyroid tissue becomes impaired, the gland may gradually lose the ability to produce adequate thyroid hormone.

Some people have elevated thyroid antibodies while maintaining normal thyroid hormone levels. Others develop mild or more significant hypothyroidism over time. Thyroid hormone treatment is generally based on thyroid function rather than antibody levels alone.

Symptoms vary from person to person and may develop gradually.

Hashimoto’s and Hypothyroidism Are Not Exactly the Same

Hashimoto’s thyroiditis describes the autoimmune process affecting the thyroid.

Hypothyroidism describes a state in which the body does not have enough thyroid hormone.

A person may have Hashimoto’s antibodies without significant hypothyroidism. Conversely, hypothyroidism can occur for reasons other than Hashimoto’s.

  • Thyroglobulin antibodies

  • Physical examination

Thyroid Hormone Treatment

When Hashimoto’s causes clinically significant hypothyroidism, thyroid hormone replacement may be necessary.

Levothyroxine, a synthetic form of T4, remains the standard treatment for hypothyroidism and is effective for many patients.

Treatment is individualized based on factors such as:

  • Cardiovascular health
  • Pregnancy status
  • Medication interactions
  • Other medical conditions

Some patients continue to report symptoms despite apparently adequate T4 treatment. In selected cases, a carefully monitored trial involving T3 may be considered after evaluating other potential causes of symptoms. Combination therapy remains an individualized decision because benefits are not consistent across all patients and inappropriate dosing may produce excessive thyroid effects.


Why Symptoms May Persist

Persistent symptoms do not always mean that more thyroid medication is needed.

Other contributing factors may include:

  • Iron deficiency
  • Vitamin B12 deficiency
  • Vitamin D insufficiency
  • Insulin resistance
  • Blood-glucose instability
  • Menopause or other hormone changes
  • Low testosterone
  • Poor sleep

Nutrition and Hashimoto’s

There is no single diet that is appropriate for every person with Hashimoto’s thyroiditis.

Gluten and Hashimoto’s

Some patients with Hashimoto’s also have celiac disease or non-celiac gluten sensitivity.

A gluten-free diet is clearly appropriate for confirmed celiac disease.

For individuals without celiac disease, the potential benefit of gluten avoidance is less predictable and should be considered individually rather than recommended automatically.

Testing may be appropriate before removing gluten, particularly when symptoms or family history suggest celiac disease.


Nutrients That Support Thyroid Physiology

Several nutrients are involved in thyroid hormone production, metabolism, and immune function.

These may include:

  • Selenium
  • Zinc
  • Iron
  • Vitamin D
  • Vitamin B12
  • Folate
  • Magnesium
  • Iodine

However, more is not always better.

Excessive supplementation may be ineffective or potentially harmful. Iodine, in particular, should not be used indiscriminately in autoimmune thyroid disease.

Supplementation should be based on dietary intake, laboratory findings, medical history, and individual clinical need.
